Requirements to acquire Identification Card for Persons of Ethiopian Origin


1. Types of Forms All forms require Adobe Acrobat reader.



1.1 Applicants of Ethiopian origin will use Form 001

1.2 The foreign national spouse of an applicant will use Form 002

1.3 Persons who wish to return an ID Card will use Form 004

2. All Applicants will have to pick up the ID cards in person at the Embassy or Consulate General

3. Requirements to obtain the Persons of Ethiopian Origin Identification Card

3.1 A valid Passport of the country of naturalization

3.2 Form 001 (A person of Ethiopian Origin Identification Card application form). Applicant’s name must appear both in English and Amharic.

3.3 $ 500.00 (USD) payable by money order only

3.4 Two copies of birth or marriage certificates, school certificate or other as requested by the Embassy

3.5 Three Passport size current color photos

4. If an applicant wishes to add children under 18 to the ID Card, they should present two copies of a legal document showing relationship (i.e. birth certificate)

5. Requirements for a foreign national spouse of an applicant who wishes to apply for an ID Card

5.1 Two copies of a marriage certificate or affidavit from a witness. Proof must show that the marriage has been valid for at least 2 years prior to application.

5.2 Form 002 (application form for the foreign national spouse of a person of Ethiopian Origin Identification Card)

6. The applicant will be notified of his or her status within 15 days of presenting the necessary documentation to the Embassy or Consulate.

7. Requirements to replace lost or damaged ID Cards

7.1 An application to replace the card (use Form 001)

7.2 A police record, or affidavit attesting to loss of card

7.3 $ 300.00 (USD) payable by money order only

8. To return the ID Card the following items are required

8.1 A signed letter stating that the card is being returned willingly

8.2 Form 004 An application to return the ID card

8.3 All ID cards that are registered to applicant’s family must also be returned.

9. The Ethiopian Origin ID Card is valid for 5 years

10. If the Ethiopian Origin ID card is not renewed after expiration for a year, it will be considered as if it were returned. A new application is needed to obtain a new one.
